The STATE OF HEALTH employee wellness program is an inclusive program designed to change your life! You are invited to enroll in this state sponsored program, which is accessible through the program portal.
STATE OF HEALTH is a no-cost, year-long program run by the State of Colorado to support your health and wellness goals. In addition to the program benefits, participants also receive a $20 per month wellness credit for joining and completing the program.
You’ll see the discount on your paystub, once you complete your first cycle.


Who can participate?
All benefits-eligible State of Colorado employees enrolled in a Cigna or Kaiser Permanente medical plan can join the STATE OF HEALTH program.

Create your STATE OF HEALTH portal account and begin today!
To participate, employees must complete the program application as the first step. Once you have submitted your application, you will receive instructions on how to create your secure portal account.
Creating your account begins with receiving the pathway link from a health coach or the portal administrator. Get started by clicking on the link you have received and complete the forms.
Complete these forms:
- Review and acknowledge the Disclaimer, Terms of Use and Privacy Policy
- Complete the program registration step
- Review and acknowledge the Fitness Channel Waiver & Release of Liability.
Once you have completed the required forms, you can start your STATE OF HEALTH program.

Why does the State of Colorado offer this benefit?
STATE OF HEALTH is a transformational program designed to improve the health of State employees through a strategic, sustainable approach that helps you manage or even reverse the associated health risks of certain chronic conditions.
With STATE OF HEALTH, your path toward improvement is the number one priority. At no-cost, you can choose one of three pathways to pursue based on your health needs and goals.
No matter the course chosen, you will have access to expertise at every level. From skill groups and online resources to physicians and health coaches, STATE OF HEALTH provides you with personalized support you won’t find anywhere else. Additionally, when you complete steps throughout the year-long program, you’ll be
eligible for a $20 monthly premium discount.
No matter the personal health journey you are on, we invite you to enroll in this new program so you too can invest in yourself and reclaim your health.
Who is eligible to participate in the program?
State of Health is currently available to State employees who are enrolled in a Cigna or Kaiser Permanente medical plan.

What is required of me to receive the $20 monthly wellness credit incentive?
In order to receive the $20 monthly wellness credit incentive, you need to complete the following five actions:
- Complete your Health Engagement Questionnaire
- Set your Goals
- Complete your Blueprint for a Healthy Lifestyle
- Schedule and Attend a Skill Group
- Visit your Primary Care Provider

What is a Skill Group and how do I schedule an appointment to attend?
Skill Group sessions are 45-minute online sessions delivered in a small group format by health coaches from Cigna and Kaiser Permanente. The Skill Groups cover a variety of important Health Topics such as Exercise, Sleep, Nutrition, Goal Setting and emotional awareness, Stress, coping and resiliency and more.
Step 1: Login to your State of Health account
Step 2: Navigate to the left blue menu bar and click on “Skill Groups”
Step 3: Navigate to the top right and click on “Book Appointment”
Note: If there is a marketing announcement in the top right corner of your screen, please click the “X” to remove this box. The “book appointment” button will be hiding under this box.
Step 4: Choose a topic that interests you and scroll down to “Confirm” your time. This will take you to a new page.
Step 5: Choose a date and time that work best for your schedule.
Please Note: To attend your Skill Group, log back into the portal and find your Appointments on your homepage. Click the link in your appointment to join the Skill Group.
What's the difference between the STATE OF HEALTH’S Self-Directed, Supportive, and Intensive pathways?
State of Health provides employees three tiers of support depending on your unique health situation.
With STATE OF HEALTH, your path toward improvement is the number one priority. At no-cost, you can choose one of three pathways to pursue based on your health needs and goals.
The Intensive Pathway is designed to provide comprehensive support for employees with two or more chronic conditions as they take courageous steps to improve their health. This pathway is available to those employees enrolled in a Cigna or Kaiser Permanente medical plan and includes the following:
- Ten 45-minute health coaching sessions
- Physician/health coach integration and collaboration
- Skill groups and discussion groups
- Portal access to risk-specific health improvement resources
Cigna members will work with the Everside Health coaching team and physicians.
KP members will work with a the KP health coaching team and physicians.
The Supportive Pathway provides in-depth support and is encouraged for employees with anxiety, depression or tobacco dependence that are working towards improving their health situation. This pathway is available to employee enrolled in a Cigna or Kaiser Permanente medical plan and includes the following:
- Ten 30-45 minute health coaching sessions
- Skill groups
- Portal access to risk-specific health improvement resources
The Self-Directed pathway is a self-paced option for employees who aren’t at risk for chronic conditions and prefer a more autonomous journey to overall wellness. includes the following:

All pathways include the option to participate in the program Action Plan and earn the wellness credit incentive.

What if I don’t complete all my action steps by the deadline for my enrollment cycle?
Just complete your action steps by the next enrollment cycle’s deadline and your discount will begin based on that enrollment cycles schedule. For example, if you started in the January enrollment cycle and you completed your action steps after March 15 but before July 15, your discount will begin in September.
Do I need to re-enroll in STATE OF HEALTH every year?
You only need to enroll in STATE OF HEALTH one time.
Can I change pathways?
Yes, if you are in the self-directed pathway and would like to change to one of the health coaching pathways (Intensive or supportive), you may make a request within the STATE OF HEALTH portal and a health coach will discuss your options for changing pathways.
If you are in a health coaching pathway and you decide you would rather be in the self-directed pathway, just tell your health coach and they will move you to the self-directed pathway.
Participants in the health coaching pathways are automatically moved to the self-directed pathway when they complete their first year in STATE OF HEALTH.
Are there other ways to earn the incentive?
For most employees, participation in STATE OF HEALTH is the only way to earn the STATE OF HEALTH insurance premium discount incentive.
Employees who have a documented ADA disability that makes it difficult to complete the action steps through the STATE OF HEALTH portal may request an alternative way to complete the action steps and earn the STATE OF HEALTH insurance premium discount.
What if I need help managing chronic conditions?
The health coaching pathways were developed specifically to help employees to better manage their chronic conditions. Be sure to list your conditions on your STATE OF HEALTH application, so your health coaching team may contact you and discuss your options. If you do not list any conditions, you will be enrolled in the self-directed pathway.
What if I don’t have any chronic conditions?
The self-directed pathway was developed to help employees prevent chronic conditions. All employees enrolled in the state’s health plans are eligible to participate in the self-directed pathway.
How does the $20 incentive show on my paycheck?
There is not a separate line item on your paycheck for the premium discount. If you were not previously receiving the discount, your health insurance premium will be $20 less than the previous month (for monthly payroll). Health insurance premiums for bi-weekly paychecks will be $10 less than the previous paycheck.
If you’re unsure whether you’ve been receiving your incentive, head to the DHR benefits page on this website: DHR SOC Benefits. Compare the rates to your payroll deduction and if it’s $20 less per month, that confirms you’ve been receiving the incentive. If you have any further questions, please contact your benefits administrator.

How do I reset my password in the Portal?
To reset your password, head to your login page by clicking this link: SOH Login Page . Click “Forgot Password”, type in the email address associated with your account, then click “Reset Your Password”. Go to the email address associated with your account and look for an email from myhealthbreak@healthbreakinc.com with instructions on how to reset your password. If you do not see an email from myhealthbreak@healthbreakinc.com or have other questions, reach out to your STATE OF HEALTH program admin at statewellness@healthbreakinc.com.
How do I remain eligible to continue receiving my incentive in the next year?
To remain eligible for your monthly incentive, you will need to complete a new Action Plan after being notified to do so. If you started receiving your incentive in January, you will receive communications around your Year 2 Action Plan in September. If you started receiving your incentive in May, you will receive communications around your Year 2 Action Plan in January of the next year. If you started receiving your incentive in September, you will receive communications around your Year 2 Action Plan in May of the next year.
When will I start receiving my incentive?
The start of your incentive will depend upon which deadline by which you completed your entire Action Plan. If you complete your Action Plan by 11/15, you will start receiving your monthly incentive the following January. If you complete your Action Plan by 3/15, you will start receiving your monthly incentive the following May. If you complete your Action Plan by 7/15, you will start receiving your monthly incentive the following September.
